Patton Park Forms

We need to all of these forms turn into the Recreation Office in order for your child to particpate in the Park Program. You can not turn them in on your first day of park.

1. Health Form

2. Copy of Child's Immunizations

3. Code of Conduct (Signed by parents and child)

4. Park Permissions Form

5. Authorization to Administer Medication (Only for children that have to have medication at Park)

To apply to be a counselor or a lifeguard please fill out the application and have two reference forms completed

and returned to the Recretaion Department.
