The Hamilton Council on Aging and Senior Center offers outreach services.

A staff member, the Coordinator of Outreach and Social Services, is available 15 hours a week to provide information and referral on local services and provide assistance on the phone, in person at the Center, or at a home visit for Hamilton residents over the age of 60.

Information provided includes:

  • in-home care
  • elder assessment
  • assisted care and long-term care facilities
  • fuel assistance
  • food stamps
  • transportation
  • support groups
  • finances
  • senior scams
  • health insurance
  • and others

The current Outreach Coordinator is certified as a SHINE counselor (Serving the Health Information Needs of Elders), and is available to assist with:

  • Medicare enrollment
  • plan changes during Open Enrollment
  • Medicare claims

As part of outreach, the Hamilton COA hosts a monthly Low Vision Group for those with low or no vision, and/or their family members. With the Director, the Outreach Coordinator develops programs to increase awareness of and to encourage use of the Senior Center and to reach out to meet the needs of Hamilton residents over the age of 60.
